Inside Cover Printing — Comic books and magazines support printing on the inside front and back cover. Your cover PDF must include a second page with the inside front and back cover artwork. Use the cover template above which already includes this second page layout. If you're creating a single-issue comic, use Saddle Stitch binding (8–80 pages). For graphic novels, use Perfect Bound (32–800 pages).

inside (gutter) margin to 1.0" or more to ensure text doesn't disappear into the binding.

edit the content. Then add it to content.opf (in both <manifest> and <spine>)
and add a link to both nav.xhtml and toc.xhtml.
